Physician, Ambulatory Palliative Care - Atlantic Health Overlook Medical Center

Ambulatory Palliative Care Physician to join a growing palliative care team across multiple outpatient sites in Northern New Jersey. Provide outpatient palliative care consultations and longitudinal follow-up, manage complex symptoms, lead goals-of-care and advanced care planning discussions, and support patients and families throughout the disease trajectory. Collaborate with oncology, primary care, hospital-based teams, and specialists to deliver interdisciplinary, patient-centered care that aligns treatment with patient values, primarily in the outpatient setting.
