Provide direct and indirect patient care as part of a team-based primary care model at Altais Medical Group. Focus on health promotion, disease prevention, health maintenance, counseling, and patient education, with annual health assessments and chronic care management. Collaborate with interdisciplinary colleagues, leverage the EHR (Epic) to manage patient care and workflows, and drive clinical initiatives for BTHealth. Maintain patient advocacy within the healthcare system and ensure communication with patients and families. Role requires board certification, CA medical licensure, and ability to participate in 24/7 call coverage; office-based with hybrid in-person and remote work as part of an integrated care team.
