Job Summary
Primary Care Physician role in Stockton, CA within a value-based care team. Deliver comprehensive primary care, evaluate acute illnesses, and create care plans with the Interdisciplinary Team (IDT). Supervise 2-3 Nurse Practitioners, oversee discharge planning for acute/long-term settings, participate in rotating call coverage, and contribute to outcomes data and quality improvement. Strong documentation in Athena to support care planning; opportunity to work in a startup-style environment with professional development benefits and equity opportunities.
Required Qualifications
- Completion of an accredited residency program (Internal Medicine, Family Practice, Emergency Medicine, or Geriatrics preferred)
- Board Certified with an active physician license in California
- State CDS Certification, DEA Registration, and experience with Medicare and Medi-Cal preferred
- Minimum of 2-3 years of experience, ideally in value-based care, primary care, or SNF settings
