Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities, performing evaluations, developing individualized plans of care, and delivering evidence-based treatment to support mobility and functional independence. Document sessions within FOX's EMR system and collaborate with an interdisciplinary team including OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff on safety and aging in place, while incorporating telehealth and virtual care when appropriate to extend access and flexibility. This role offers the autonomy to create and manage your own treatment schedule within a high-growth, clinician-operated practice dedicated to abolishing ageism and improving quality of life for older adults.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Ability to exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Ability to perform CPR
- Ability to provide moderate to maximum assistance with patient transfers and bed mobility
- Ability to reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities
- Ability to drive to patient homes and senior living communities as part of the Geriatric House Calls model
- Ability to wear PPE when required

- Part-time Physical Therapist – In-Home & Senior Living Care (salaried with benefits eligibility based on minimum hours requirement)
- PRN / FOX Flex Physical Therapist – Home & Community Visits (pay per unit compensation)
