Job Summary
Physical Therapy Assistant to join a primary care team delivering rehabilitative services to homebound patients across SC/GA/FL, performing in-home, ALF/ILF visits under licensed PT supervision; responsibilities include implementing treatment plans, guiding patients through therapeutic exercises, documenting progress, coordinating with physical therapists and healthcare team, ensuring a clean therapy environment, and participating in discharge planning with follow-up instructions; requires associate degree and current licensure eligibility, CPR, bilingual ability preferred; prior specialized PT areas (orthopedics, pelvic health, neurology, geriatrics) and EMR proficiency encouraged; salary-based with monthly bonus and comprehensive benefits.
Required Qualifications
- Associate degree from an accredited Physical Therapy Assistant program
- Current licensure or eligibility for licensure as a Physical Therapy Assistant in the state of practice
- CPR certification
- Strong interpersonal and communication skills
- Ability to work effectively as part of a multidisciplinary team
- Excellent organizational and time management skills
- Compassion and empathy for patients undergoing rehabilitation
