Job Summary
Deliver individualized treatment plans established by the supervising Physical Therapist, instruct and safeguard patients through therapeutic exercises and functional activities, and document visits accurately within FOX’s EMR system. Work in-home and in senior living settings as part of an interdisciplinary team with PTs, OTs, SLPs, and Fitness Specialists; enjoy mentorship from supervising PT and opportunities to grow clinical skills, caseload, and career. Participate in coordinating patient care, staying connected with patients and referral sources, and contributing to a collaborative, compassionate care environment.
Required Qualifications
- Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain
- Graduate of an accredited Physical Therapist Assistant program
- Current CPR certification
- Basic computer skills; proficient in Microsoft Office
- Physical Requirements: lift/move patients, standing, walking, driving to patient homes, PPE as required
