Job Summary
Deliver individualized treatment plans under the supervision of a Physical Therapist, guiding patients through therapeutic exercises and functional activities in-home and within senior living communities. Collaborate within an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists) to implement evidence-based care, document visits accurately in FOX’s EMR, and coordinate coverage as needed. Build trusting, one-on-one relationships with older adults to improve strength, mobility, and confidence, with mentorship from supervising PT and opportunities for skill growth in geriatric home-based rehabilitation.
Required Qualifications
- Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain
- Graduate of an accredited Physical Therapist Assistant program
- CPR certification
