Job Summary
Conduct physical therapy treatments and provide exercise instruction aimed at improving patient well-being while following the established plan of care. Instruct patients and caregivers on active and passive exercises, gait training, and functional mobility. Observe, record, and report patient responses to treatment and condition changes to the supervising PT, nurse supervisor, and physician. Coordinate care with the healthcare team and make recommendations for changes in the plan of care. Follow all state-specific laws governing physical therapy in home care, performing only procedures within your qualified scope. Work a weekend rotation in any home situation regardless of client demographics.
Required Qualifications
- Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice
- Current driver's license and vehicle insurance
- Access to a dependable vehicle
- Public transportation
- Available to work a weekend rotation
- Ability to pass a drug test
Desired Qualifications
- Current CPR certification
- Ability to complete CPR certification within 90 days of hire
- Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility
