Job Summary
Perform treatment and provide exercise instruction and patient education aimed at improving patient well-being while following the established plan of care. Instruct and aid patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training. Observe, record, and report patient responses to treatment and condition changes to the supervising PT, nurse supervisor, and/or physician. Coordinate care with the healthcare team and make recommendations for changes in the plan of care. Follow all state-specific laws governing physical therapy in home care and perform only procedures qualified and trained to perform. Available to work a weekend rotation. Requires a current driver's license and vehicle insurance or access to public transportation.
Required Qualifications
- Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice
- Current driver's license and vehicle insurance
- Access to a dependable vehicle
- Public transportation
- Available to work a weekend rotation
- Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility
- Candidates are required to pass a drug test before beginning employment
Desired Qualifications
- Current CPR certification
- Ability to complete CPR certification within 90 days of hire
