Job Summary
Conduct physical therapy services for patients recovering from injuries or illness under the guidance of a licensed Physical Therapist in a home health setting. Assist patients in moving better, feeling better, and staying active while utilizing HCHB electronic medical record. Requires an Associate degree in Physical Therapy, an active state license, and at least 12 months of recent home health experience. Schedule includes days, 8 hours per day, Monday through Friday for a 13-week contract in Alpena, MI.
Required Qualifications
- Associate degree in Physical Therapy from an accredited educational program
- Active State License
- BLS Certification from AHA or ARC
- At least 12 months of Home Health experience within the past 3 years
- Experience with HCHB (HomeCare HomeBase) electronic medical record
- Previous experience providing care in a home health setting
Desired Qualifications
- Additional certifications or training in geriatric or orthopedic home health care
