Job Summary
Perform treatment, provide exercise instruction, and deliver patient education aimed at improving well-being while following the established plan of care. Instruct patients and caregivers on active and passive exercises, gait training, functional activities, and safety protocols. Observe, record, and report patient responses to treatment and condition changes to supervising PTs, nurses, and physicians. Coordinate care with the broader healthcare team and make recommendations for plan-of-care adjustments. Adhere to state-specific laws governing physical therapy in home care, performing only procedures within your qualified scope. Work a weekend rotation in various home situations regardless of client demographics.
Required Qualifications
- Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice
- Current driver's license and vehicle insurance
- Access to a dependable vehicle
- Public transportation
- Available to work a weekend rotation
- Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility
- Candidates are required to pass a drug test before beginning employment
Desired Qualifications
- Current CPR certification
- Ability to complete CPR certification within 90 days of hire
