Job Summary
Deliver physical therapy treatments and related duties as delegated by the registered physical therapist to program participants on acute, restorative, and maintenance levels. Communicate participant changes and progress per treatment plan on a regular basis to the Registered Physical Therapist while maintaining proper documentation according to state policies, federal PACE regulations, and EMR records. Perform non-evaluative ongoing data recording as specified by the planned treatment program and manage multiple priorities within the scope of practice. Work M-F, 8a-5pm with occasional weekends and evenings at a rate of $31-$34 per hour.
Required Qualifications
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- Must have a valid driver's license and have means of transportation, to assist participants in their homes or other settings.
- Must clear the background check.
- Graduate of a Physical Therapist Assistant curriculum, with a active PTA license in Kentucky.
- Minimum of one-year experience working with a frail or elderly population required.
- Good working knowledge of physical, psychosocial and behavioral needs of the older population.
- Effective listening, and written and oral communication skills.
- Demonstrated ability to problem solve and apply good judgment.
- Demonstrated ability to efficiently and effectively manage multiple priorities.
- Working knowledge of practice boundaries for Physical Therapist and Physical Therapy Assistant (P.T.A.).
- Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
- Must meet a standardized set of competencies for the specific position description established by Senior CommUnity Care and in accordance with CMS before working independently.
