Job Summary
Provide direct physical therapy interventions as planned, delegated, and supervised by the Physical Therapist while recording treatments and noting progress in medical records. Develop and implement individualized treatment plans to increase resident independence, communicate condition changes to the interdisciplinary team, and ensure compliance with mandatory education programs. Perform screening rounds to assess client needs and environmental safety, coordinate training with Restorative Nursing and other care staff, and educate patients, caregivers, and community partners. Submit accurate billing records and supporting clinical documentation. Maintain an active, unencumbered Physical Therapy Assistant license and an associate's degree. Join our in-house team at Tampico Healthcare Center in Walnut Creek, CA, as a part-time role emphasizing superior quality care in a quiet, intimate environment.
Required Qualifications
