Job Summary
Deliver direct physical therapy services under the direction of a licensed Physical Therapist in a home health setting. Provide individualized care to help patients reduce pain, improve mobility, and restore function through exercises, gait training, balance work, and activities of daily living; utilize therapeutic modalities (heat, cold, ultrasound, electrical stimulation) as appropriate; educate patients and caregivers on exercises and safety; document progress in the EMR and participate in case conferences and care coordination with the interdisciplinary team. Hybrid collaboration with therapists, nurses, and care teams to ensure quality care and ongoing professional development. Requires PTA program graduation, state licensure, CPR certification, and a valid driver’s license; 1+ year of experience is preferred. Location: Bradenton, FL with field-based, home-health duties.
Required Qualifications
- Graduate of an accredited Physical Therapy Assistant (PTA) program
- Current PTA license or certification as required by state law
- 1+ year of experience preferred (home health experience a plus)
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
