Job Summary
Conduct in-home physical therapy visits weekly for clients with chronic and terminal illnesses, following physician plans of care. Complete visits lasting 1-2 hours, documenting services, observations, and progress on designated forms within seven days. Report changes or schedule adjustments to therapists and nursing supervisors daily. Participate in interdisciplinary activities, client care conferences, and staff meetings while promoting referrals to other disciplines. Adhere to safety protocols, infection control practices, and confidentiality standards for all client interactions.
Required Qualifications
- Current license, registration or certification to practice as a Physical Therapy Assistant in the state(s) of employment or be qualified to take the physical therapy examination.
- Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association.
- Minimum of six (6) months experience as a Physical Therapy Assistant.
- Demonstrated written, verbal communication skills, and interpersonal skills
- Licensed driver with clean driving record and insured in accordance with state and/or Agency requirements.
