Physical Therapy Assistant (PTA) - Home Health - PRN

Deliver direct physical therapy services under the direction of a licensed Physical Therapist; follow individualized plans of care to help patients reduce pain, improve mobility, and restore function; assist patients with therapeutic exercises, gait training, balance, and functional mobility; support transfers, strengthening, and activities of daily living; utilize modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate; educate patients and caregivers on exercises, safety, and use of assistive devices; observe and report progress; maintain EMR documentation; communicate with the Physical Therapist and care team; collaborate with nurses and therapists; participate in team meetings and ongoing training; uphold clinical standards and contribute to quality improvement.
