Physical Therapy Assistant (PTA) - Home Health - Full Time

Deliver direct physical therapy services under the direction of a licensed Physical Therapist, assisting patients with therapeutic exercises, gait training, balance, and functional mobility. Provide hands-on care for transfers, strengthening, and activities of daily living while utilizing modalities such as heat, cold, ultrasound, and electrical stimulation. Educate patients and caregivers on exercises, safety, and assistive device use, then observe and report progress to maintain accurate EMR documentation. Collaborate with nurses, therapists, and interdisciplinary teams to ensure coordinated care and participate in case conferences.
