Provide skilled physical therapy services in patient homes under a Physical Therapist's plan of care, performing prescribed procedures to relieve pain and restore function. Educate patients and families on therapy techniques, safety measures, and proper use of equipment like wheelchairs and walkers. Assess functional problems, participate in case conferencing, and complete all clinical documentation within 24 hours of visits. Drive to assigned locations and handle on-call rotations while maintaining compliance with state scope of practice and Medicare guidelines.
