Job Summary
Home Health PTA provides home-based physical therapy to patients, coordinating with an interdisciplinary team, performing assessments, guiding patient exercises, educating patients and families on the plan of care, and adapting treatment plans to improve mobility and function in a home setting. Qualified candidates hold an accredited PTA program degree, Wisconsin PTA licensure, CPR, and have at least 1 year of PTA experience (preferably in home health). Will operate within home health regulations and EMR systems, with mileage reimbursement and a comprehensive benefits package. The role emphasizes patient-centered care, documentation of progress, and collaboration with RNs, LPNs, CNAs, HHAs, OTs and SLPs to achieve care goals.
Required Qualifications
- Graduate of an accredited Physical Therapist Program
- active PTA license in Wisconsin
- Minimum of 1 year of physical therapy assistant experience (ideally in home healthcare)
- CPR certification
- Knowledge of state and federal home health regulations
- Good clinical judgement, interpersonal skills, dexterity and compassion
