Job Summary
Provide home-based physical therapy to patients in Fort Wayne, IN, as part of a home health team. Observe movements, conduct assessments, document progress and modify the care plan as needed; assist patients with exercises and hands-on therapy to improve mobility and reduce pain; educate patients and families on the plan of care, goals, exercises, and self-care; collaborate with RNs/LPNs/CNAs/HHA/Ot/SLP to achieve patient goals. Requires graduation from an accredited PTA program and Indiana PTA license, CPR certification, and at least 1 year of PTA experience (preferably in home health).
Required Qualifications
- Graduate of an accredited Physical Therapist Program and active PTA license in Indiana
- Minimum of 1 years of physical therapy assistant experience, ideally in home healthcare
- CPR certification
- Knowledge of state and federal home health regulations
