Job Summary
Conduct weekly in-home physical therapy visits lasting 1-2 hours for clients with chronic and terminal illnesses resulting from prior nuclear weapons work. Provide services and education following the physician's Plan of Care, documenting progress and recommendations within seven days. Report changes or schedule adjustments to the therapist and nursing supervisor daily. Participate in interdisciplinary conferences, staff meetings, and continuing education programs while maintaining strict confidentiality and safety compliance. Perform additional duties as needed and adhere to Agency policies, Medicare, CHAP, and Joint Commission standards.
Required Qualifications
- Current license, registration or certification to practice as a Physical Therapy Assistant in the state(s) of employment or be qualified to take the physical therapy examination
- Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association
- Minimum of six (6) months experience as a Physical Therapy Assistant
- Demonstrated written, verbal communication skills, and interpersonal skills
- Licensed driver with clean driving record and insured in accordance with state and/or Agency requirements
- Must be available for weekly visits
- Must be available for visits lasting 1-2 hours per visit
