Physical Therapy Assistant (PTA) (58145)

Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury. Conduct assessments, document progress, and modify care plans as needed. Assist patients with exercises and hands-on therapy to manage pain and increase mobility. Educate patients on their plan of care, established goals, and self-care while coaching families on safely assisting with exercises. Work as part of a home health team including RNs, LPNs, CNAs, HHAs, OTs, and SLPs focused on the patient's plan of care and goals. This part-time role (2-3 days/week) offers market competitive visit rates, mileage reimbursement, PTO, and a 4% 401k match.
