Physical Therapy Assistant - PRN

Assess referred patients requiring rehabilitation services and provide direct patient care to evaluate medical conditions, functional capabilities, and limitations. Establish and administer treatment programs with specific goals determined by patient capacity under physician direction, while setting realistic objectives. Document patient conditions, maintain weekly progress notes, manage recertification and discharge documentation, and educate families on appropriate physical therapy methods. Collaborate with all disciplines to deliver patient-centered care and perform duties assigned by the Director of Therapy while maintaining compliance with confidentiality and federal resident rights. Requires a Bachelor of Science in Physical Therapy, successful National Certification Examination, and current state licensure.
