Job Summary

Deliver physical therapy services under the direction of a Physical Therapist, implementing treatment plans and assessing client effectiveness while documenting all visits in compliance with agency billing and reimbursement requirements. Collaborate with the multidisciplinary team to communicate client issues, coordinate care plans, and facilitate referrals for medical, social, and emotional factors. Adapt treatments to the unique home setting, fabricate and adjust adaptive equipment, and teach discharge therapy programs to clients and families. Independently manage case loads by prioritizing urgent needs, scheduling home visits, and organizing weekly meetings, while requesting supervision or continuing education as needed. Maintain strict confidentiality of client information, adhere to infection control and safety protocols, and participate in team reassessments and program planning.

Required Qualifications

  • Associate degree in Physical Therapy from an institution of higher learning accredited by the Commission on Accreditation in Physical Therapy Education
  • Current, unencumbered (State Employed) License as a Physical Therapy Assistant
  • National Provider BLS
  • American Heart Association (Vendor Managed) AK: Required upon request
  • Valid driver's license and auto insurance
  • Ability to lift and move objects up to 50 pounds
  • Close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
  • Ability to write reports, business correspondence, and procedure manuals
  • Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
  • Strong written and verbal communications
  • Ability to communicate in English effectively both verbally and in writing
  • Ability to understand, read, write, and speak English
  • Knowledge of relevant rehabilitation therapy techniques and procedures in respective discipline
  • Demonstrated knowledge and skills necessary to provide appropriate care to clients ranging in age from adolescents to geriatrics including knowledge of the principles of growth and development over the life span, possession of the ability to assess data reflective of the client's status and be able to interpret the appropriate information needed to identify each client's requirements relative to his or her age specific needs
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
  • Ability to compute rate, ratio, and percentage

Desired Qualifications

  • 1 year of Physical Therapy Assistant experience in a home care or rehabilitation setting
