Physical Therapy Assistant Long Term Care Per Diem

Implement individualized treatment plans and provide physical therapy treatments to facilitate client independence under the supervision of licensed Physical Therapists. Carry out established plans of care, adjust procedures based on patient status changes, and organize treatment groups in consultation with the primary therapist. Provide objective documentation of patient care, education, and units of service in the electronic medical record while maintaining confidentiality and adhering to infection control policies. Act as an assistant during tests and evaluations, collaborate with the rehabilitation team to develop programs, and participate in quality improvement initiatives and required meetings. Rotate and flex according to staffing needs within HSL Therapy Programs.
