Job Summary
Provide treatment, tests, client education, and appropriate interventions to assist adult and geriatric patients in attaining or maintaining optimal physical well-being as delegated by a licensed Physical Therapist. Perform home health visits throughout Lebanon, NH on a per-visit basis for full-time or part-time schedules. Maintain current New Hampshire licensure or certification and hold at least one year of supervised work experience. The role requires a two-year college degree from an APTA-approved program or equivalent experience with a satisfactory proficiency exam. Required Qualifications
- Current licensure or certification in the state of New Hampshire
- Graduation from a two-year college level program approved by the American Physical Therapy Association (indicated by school transcript or diploma)
- One year of appropriate experience as a Physical Therapist Assistant (PTA) AND a satisfactory grade on a proficiency exam conducted, approved or sponsored by the U.S. Public Health Service (for initial qualification before December 31, 1977)
- At least one year's work experience under the supervision of a qualified Physical Therapist
