Physical Therapy Assistant, Home Care, Westchester

Provides physical therapy interventions under the supervision of a Physical Therapist (PT), follows the PT plan of care, documents care and progress, conducts PT visits in patients' homes or care facilities, collaborates with Supervising Therapist, and participates in interdisciplinary meetings. Responsibilities include prescribing, demonstrating, and verifying therapeutic activities; monitoring patient progress with PT; coordinating care with family members and paraprofessionals; assisting with transfers and use of durable medical equipment; and adhering to VNS Health policies. Travel to patient homes and facilities is required, with attention to varied environments and safety considerations. The PTA works within the scope of practice with required re-assessment by PT and co-visits on initial PTA visits.
