Job Summary
Deliver physical therapy interventions to designated patients under the supervision of a Physical Therapist, following established plans of care and modifying procedures as appropriate. Assist patients with therapeutic activities, positioning, transferring, and lifting, while instructing family members on durable medical equipment use. Document all care provided and monitor patient progress through observations, following up with the supervising therapist per VNS Health policy. Travel to patients' homes and facilities using approved transportation, managing assigned caseloads and participating in interdisciplinary team meetings and caseload reviews.
Required Qualifications
- New York State Certification as Physical Therapist Assistant
- Valid driver's license
- Associate's Degree as Physical Therapist Assistant from a New York State approved program
- Minimum of two years experience in clinical patient care
