Job Summary
Deliver individualized treatment plans established by your supervising Physical Therapist; instruct, motivate, and safeguard patients through therapeutic exercises and functional activities, observing and tracking their progress in-home and in senior living settings; document visits accurately in FOX’s EMR system; stay connected with patients, POAs, and community referral sources, and help coordinate coverage when needed; work in Franklin, MA as part of an interdisciplinary care team (PT, OT, SLP, Fitness Specialists) with mentorship from a supervising PT and opportunities to grow clinical skills, caseload, and career; embody FOX’s Geriatric House Calls model and focus on independence, wellness, and longevity.
Required Qualifications
- Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain
