Job Summary
Provide rehabilitation therapy procedures to patients under the direction and supervision of a physical therapist. Assist with patients and caregiver education. Maintain a driver's license and current Basic Life Support certification as required for this home care role. Work full-time, Monday through Friday, from 7am to 6pm. This position is part of Corewell Health's Rehab Rockford team, which supports a network of hospitals, outpatient locations, and post-acute facilities serving over 1.2 million members through a commitment to equitable health care.
Required Qualifications
- Associate's Degree accredited physical therapy assistant program
- LIC-Physical Therapist Assistant - STATE_MI State of Michigan Upon Hire
- CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days
- CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days
- LIC-Driver's License - STATE_MI State of Michigan Home Care Business Lines
- compliance with the MMR, Varicella, Tdap, and Influenza vaccine requirement
