Job Summary
Provide in-home physical therapy to home-bound patients in Westerville and surrounding areas as a full-time Physical Therapist Assistant. Responsibilities include conducting routine visits with effective therapeutic interventions, collaborating with other disciplines to deliver comprehensive care, monitoring and adjusting the patient’s plan of care to potentially reduce rehospitalization, and educating family members about care processes. The role supports a stable caseload with orientation, meetings, and CEU participation as part of ongoing professional development. Flexible hours and a focus on quality patient outcomes are emphasized, with care delivered in the patient’s home and in collaboration with the care team.
Required Qualifications
- Ability to perform routine visits utilizing effective therapeutic interventions
- Work with other disciplines to give the best care needed for each patient
- Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed
- When appropriate teach and counsel family members about care and processes for disease/injuries
- Attend orientation, meetings, and CEUs
