Physical Therapy Assistant

Physical Therapy Assistant for an in-house Skilled Nursing Facility provides therapy services to residents in accordance with standards of practice, completes compliant CMS documentation in NetHealth EMR, and collaborates with an interdisciplinary team to evaluate residents, develop and implement short- and long-term treatment plans, and communicate findings to residents and families. Responsibilities include screening residents, conducting specialized evaluations, ensuring productive therapy sessions, and maintaining documentation and billing per procedures. Requires an Associate's Degree in Physical Therapy and an active PT license in the applicable state(s); ongoing continuing education to maintain licensure. Location: Plainfield, NJ; full-time role with in-person duties.
