Physical Therapy Assistant

Provides skilled physical therapy services under the direction of a Physical Therapist in patients' homes, educates patients and families, ensures safety, completes accurate documentation consistent with Medicare and agency guidelines, and collaborates with physicians, therapists, and coworkers. Responsibilities include performing PT procedures and diagnostic/functional assessments, contributing to care plans to improve functional status, educating staff and patients on therapy techniques and use of equipment, participating in case conferences and on-call duties, and supporting coordination of care across the team.
