Physical Therapy Assistant

Provides skilled Physical Therapy Assistant services under the direction of a Physical Therapist in patients' homes, including performing PT procedures, documenting treatment and patient progress, educating patients and families, and coordinating with the care team. Travel to patients’ homes is required; duties include driving to visits, applying therapy modalities, promoting independence, documenting all aspects of care within Medicare guidelines, and participating in case conferences and on-call requirements. The role emphasizes assessment of functional status, selection and modification of therapy techniques, and collaboration with physicians, therapists, and staff to achieve patient-centered outcomes. Requires effective communication, patient education, adherence to plan of care, and strict documentation practices while providing compassionate, home-based rehabilitation.
