Job Summary
Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury. Conduct assessments, document progress, and modify care plans while assisting patients with exercises and hands-on therapy to manage pain and increase mobility. Educate patients and coach families on established goals, exercises, and self-care within a multidisciplinary team focused on individualized patient plans. Maintain knowledge of state and federal home health regulations and exercise strong clinical judgment. Full-time schedule with market competitive visit rates and mileage reimbursement.
Required Qualifications
- Graduate of an accredited Physical Therapist Program
- Active PTA license in Florida
- Minimum of 1 years of physical therapy assistant experience
- CPR certification
- Knowledge of state and federal home health regulations
- Good clinical judgement
- Strong interpersonal skills
- Dexterity
- Compassion
