Job Summary
Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury. Conduct assessments, document progress, and modify care plans while observing movements and assisting with exercises to manage pain and increase mobility. Educate patients and coach families on self-care, therapy goals, and safe exercise techniques as part of a multidisciplinary team including RNs, LPNs, and OTs. Maintain knowledge of state and federal home health regulations and exercise strong clinical judgment. Requires an active Indiana PTA license, one year of experience, and CPR certification.
Required Qualifications
- Graduate of an accredited Physical Therapist Program
- Active PTA license in Indiana
- Minimum of 1 years of physical therapy assistant experience
- CPR certification
- Knowledge of state and federal home health regulations
- Good clinical judgement
- Strong interpersonal skills
- Dexterity
- Compassion
