Job Summary
Conduct home-based physical therapy treatment, exercise instruction, and patient education while strictly adhering to the supervising PT's established plan of care. Instruct patients and caregivers on active/passive exercises, gait training, ADLs, transfers, and safety protocols. Observe, record, and report patient responses and condition changes to the supervising PT, nurse supervisor, and physician. Coordinate care with the broader healthcare team and make recommendations for plan adjustments as needed. Perform only procedures within your qualified scope and comply with all state-specific home care laws. Available to work a weekend rotation with a current driver's license and vehicle access.
Required Qualifications
- Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice
- Current driver's license and vehicle insurance
- Access to a dependable vehicle
- Public transportation
- Available to work a weekend rotation
- Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility
- Candidates are required to pass a drug test before beginning employment
Desired Qualifications
- Current CPR certification
- Ability to complete CPR certification within 90 days of hire
