Job Summary
Conduct home-based physical therapy treatment and provide exercise instruction, muscle re-education, and patient education to improve well-being while following the supervising PT's established plan of care. Observe, record, and report patient responses to treatment and condition changes to the supervising PT, nurse supervisor, and physician. Coordinate care with the healthcare team and instruct patients, families, and caregivers on therapy within the scope of practice. Perform active and passive exercises, gait training, ADL, transfer, safety, and prosthetic training in any home situation. Maintain a current driver's license and vehicle insurance or access to public transportation. Available to work a weekend rotation. Requires current licensure to assist in physical therapy practice.
Required Qualifications
- Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice
- Current driver's license and vehicle insurance
- Access to a dependable vehicle
- Public transportation
- Available to work a weekend rotation
- Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility
- Candidates are required to pass a drug test before beginning employment
Desired Qualifications
- Current CPR certification
- Ability to complete CPR certification within 90 days of hire
