Job Summary
Conduct skilled therapy visits in patients' homes across O'ahu, performing PT evaluations, follow-up assessments, and developing fall prevention strategies. Implement mobility, balance, and strengthening programs while educating patients and family caregivers. Complete accurate Medicare-compliant documentation for each visit. Requires current Hawaii PT license, 1+ year clinical experience, and reliable transportation. Flexible per diem schedule with $80 per visit compensation.
