Job Summary
Develop and implement individualized care plans for homebound patients. Evaluate and provide interventions to improve balance, gait, strength, mobility, and functional independence. Participate in interdisciplinary team meetings and direct Physical Therapy Assistants and Home Health Aides. Requires a Virginia PT license, one year of home health experience, and a clean driving record. Full-time and part-time roles available in Blacksburg, VA.
Required Qualifications
- Licensed to practice Physical Therapy in the Commonwealth of Virginia
- Minimum 1 year of home health experience
- Valid driver's license and clean driving record
- Successful completion of a background check
