Job Summary
Conduct home, ALF, ILF, clinic, and telehealth visits for primary care patients throughout the Augusta area. Obtain complete health and psychosocial histories, perform physical assessments, provide health education and counseling, and arrange referrals to the health team and community resources. Review medical diagnoses, prognosis, procedures, and clinical course while utilizing appropriate reporting mechanisms and obtaining necessary authorizations. Work a full-time 8-hour schedule (8:00AM-5:00PM, Monday-Friday) with travel between offices and facilities required.
Required Qualifications
- Successful completion of a Physical Therapy training program
- Certified through the therapy state board
- Current license to practice in the state of SC
- Current CPR Certificate
- Ability to utilize electronic documentation
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, and other places
- At least one year experience in the specialty field
Desired Qualifications
- Previous experience in healthcare setting
- Experience and/or interest in Pelvic Health
