Job Summary

Required Qualifications
- Licensed Physical Therapist in state(s) of practice
- State licensure as Physical Therapist

- Valid driver’s license if applicable
- Drug screen and pre-employment background screening as a condition of employment
- Obtain and maintain department-specific competencies and certifications
