Job Summary
Conduct comprehensive physical therapy evaluations and develop individualized treatment plans to improve mobility, strength, balance, and functional independence for adult and geriatric populations. Implement therapeutic interventions and monitor patient progress while adjusting care plans as needed. Collaborate with physicians, nurses, and rehabilitation staff to coordinate interdisciplinary care across outpatient, acute, inpatient, or skilled nursing settings. Educate patients and caregivers on exercises, mobility techniques, and fall prevention strategies. Ensure compliance with clinical standards, safety protocols, and regulatory requirements. Required Qualifications
- Bachelor's degree in Physical Therapy
- Valid California Physical Therapist License
- proof of license eligibility
Desired Qualifications
- Master's or Doctorate degree
- Minimum of two (2) years of clinical experience
- CPR and First Aid certification
- Outpatient rehabilitation experience
- Acute care experience
- Inpatient rehabilitation experience
- Skilled nursing facility (SNF) experience
- Strong experience working with geriatric patients
- Strong geriatric rehabilitation experience
- Experience in outpatient or acute care settings
- Open to interdisciplinary, holistic care environments
- Strong commitment to functional, patient-centered outcomes
