Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities, performing evaluations, developing individualized plans of care, and delivering evidence-based treatment to support mobility and functional independence. Document sessions within FOX's EMR system and collaborate with interdisciplinary teams including OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff on safety and aging in place while offering telehealth or virtual care when appropriate. This role requires a valid Florida PT license, EMR proficiency, and ability to lift up to 50 lbs occasionally. Full-time, part-time, and PRN positions are available in Winter Park, FL, with H1B sponsorship for qualified clinicians.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Performing CPR
- Providing moderate to maximum assistance with patient transfers and bed mobility
- Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
- Driving to patient homes and senior living communities as part of the Geriatric House CallsTM model
- Wearing PPE when required
- Must be cleared through the Florida Background Check Clearinghouse
