Job Summary
Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, memory care) within FOX Rehabilitation’s Geriatric House Calls model. Conduct evaluations, develop individualized plans of care, and deliver evidence-based treatment to improve mobility, independence, and quality of life. Document in FOX’s EMR, collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists), and educate patients, caregivers, and staff on safety and aging in place. Telehealth/virtual care options available to extend access. Flexible schedule with autonomy to manage treatments; opportunities to work in-home and in senior living settings. Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
