Job Summary
Evaluate patients' physical abilities and limitations through comprehensive assessments, then develop individualized treatment plans based on patient needs and goals. Implement therapeutic exercises and interventions to enhance mobility, strength, and function while educating patients and families on treatment plans, home exercises, and injury prevention strategies. Collaborate with healthcare professionals to ensure coordinated care and effective discharge planning, and maintain accurate documentation of patient progress and treatment outcomes. Provide home community care as needed to support patients' recovery in their own environments. Stay updated on the latest advancements in physical therapy practices and techniques.
Required Qualifications
- Strong understanding of anatomy, physiology, and medical terminology
- Knowledge of physiology, anatomy, medical terminology, and discharge planning
- Ability to work collaboratively in a fast-paced environment while maintaining a focus on quality care
Desired Qualifications
- Proven experience in patient care within a clinical setting
