Job Summary
Physical Therapist with full-time schedule in Fresno, CA for BoldAge PACE. Provides PT services to PACE participants, conducts in-person assessments, develops and implements treatment plans, educates participants and families, collaborates with the IDT, documents care, trains staff, and participates in quality improvement projects; requires advanced PT degree, state licensure, and experience on an interdisciplinary team, with a sign-on bonus of $5,000.
Required Qualifications
- Advanced degree in Physical Therapy from an accredited school of Physical Therapy
- Current license to practice Physical Therapy in the state
- Current driver’s license and proof of auto insurance
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred
