Job Summary
Plan, organize, and implement physical therapy services for PACE participants through in-person assessments, treatment program administration, and home safety evaluations. Collaborate with the interdisciplinary team to develop comprehensive care plans, document clinical services accurately, and educate participants and families on home programs and procedures. Train staff on body mechanics, equipment use, and transfer techniques while maintaining a safe working environment and adhering to OSHA guidelines. Participate in daily team meetings, quality improvement projects, and continuing education to support participant independence and community reintegration.
Required Qualifications
- Advanced degree in Physical Therapy from an accredited school of Physical Therapy
- Current license to practice Physical Therapy in the state
- Current driver's license and proof of auto insurance
Desired Qualifications
- Bilingual and fluent in Hmong preferred
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred
