Job Summary
Plan, organize, and implement physical therapy services for PACE participants, including initial and ongoing in-person assessments, adaptive equipment evaluations, and home safety modifications. Administer treatment programs consistent with recognized standards, document evaluations and care plans accurately, and educate participants and families on home programs and safety procedures. Collaborate with interdisciplinary team members to develop comprehensive care plans, participate in daily IDT meetings, and train staff on body mechanics and functional abilities. Provide care in centers, clinics, contracted facilities, and participants' homes while maintaining OSHA safety guidelines and confidentiality. Full-time Monday-Friday schedule with a $5,000 sign-on bonus.
Required Qualifications
- Advanced degree in Physical Therapy from an accredited school of Physical Therapy
- Current license to practice Physical Therapy in the state
- Current driver's license
- Proof of auto insurance
Desired Qualifications
- Bilingual and fluent in Hmong
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting
- 1 year of experience working with a frail or elderly population
