Job Summary
Assess, plan, and implement physical therapy services for PACE participants through in-person evaluations, home safety modifications, and adaptive equipment recommendations. Administer treatment programs consistent with recognized standards while collaborating with the interdisciplinary team to develop comprehensive care plans and coordinate 24-hour delivery. Educate participants and families on home programs, train staff on body mechanics and transfers, and document all clinical services accurately. Maintain safety protocols, participate in daily team meetings, and engage in quality improvement projects to support seniors in living meaningful, independent lives at home.
Required Qualifications
- Advanced degree in Physical Therapy from an accredited school of Physical Therapy
- Current license to practice Physical Therapy in the state
- Current driver's license and proof of auto insurance
Desired Qualifications
- Bilingual and fluent in Hmong preferred
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred
